Executive Summary
The Rural Health of Virac encountered difficulty in recording the patient data, its maintanance, and the retrieval of patient's record. Furthermore, it was tasking and time-consuming on the part of their dealing with this kind of transaction resulting into an inefficient delivery of services. Likewise, problems were also encountered in terms of shortage in office supplies, such as papers, folder, etc as well as filing cabinets or boxes which occupy bigger space inside the clinic. This study presented the Virac RHU - Clinic Support Sytem, an information system that register patient's data about his medical history, diagnosis, prescription, and generates patient record. The study used the SDLC as the development methodology. A simple Bus Topology was used for the system's network topoogy. The study also presented certain graphical representation of the processess sus as a context diagram and a DFD ( data flow diagram). Likewise, an IPO addtion, a Use Case Diagram was also used to show each user's interactions with the system. In to the evaluation of the system, a five-point rating scale questionnaire was used whose respondents were the RHU personnel, wherein the respondents would give a rating og 5-1 to the corresponding ten question. From capable management of patients record, to queries and to report generation, the system alsos enhanced the physician's decision-making in giving diagnosis and prescription to the patient. With a rating of "Very Good" from the respondents, the system had realized most of its objective and had met the rural health unit's main concern of patient record management.
